Are you considering becoming an EMT or Paramedic? Working as an EMT/Paramedic can be a very rewarding career, but receiving and maintaining your license will require continuing education and continual renewal of your certifications. Becoming Licensed/Certified as an EMT varies by each State, and most states require that you to initially be certified by the Nationally Registry of Emergency Medical Technicians (NREMT.ORG) and be CPR certified before you can apply your State license. Here is a list of what you may need to do to become certified as an EMT in your state:
- Take a State approved EMT course (approximately 1-4 months)
- CPR Certification (1-2 days)
- Pass your State approved EMT Course and Practical Exams
- Sign up to take, and pass the NREMT Practical Exams
- Receive your ATT (Authorization to Test) from the NREMT, and pass the NREMT computer based test (CBT)
- Receive your NREMT Certificate (valid for approximately 2 years)
- Apply to be Licensed as an EMT in your State (may require a fee, background check, etc…)
Each State is different, so make sure you look into your State’s requirements for becoming licensed as an EMT. (There are helpful links for each State at the bottom of this article). You will not be able to work as either an EMT or Paramedic without a STATE LICENSE (difference between EMT and Paramedic). You cannot work as an EMT with only your NREMT certificate, Your State License will be your license to work as an EMT in that State.
Receiving your NREMT certification is usually the hard part, once you have that, it is usually relatively easy to get a State EMT license (may need to pay a fee, background check, application process, etc…), but some States are a little more difficult to receive a license than others (check the State links at the bottom of this page).
Understand the Difference Between an EMR, EMT, AEMT, and Paramedic
Some of the websites that I have seen do not properly distinguish between all of these when they describe”how to become an EMT”, You can read my post about the difference between a paramedic and EMT to help contrast the difference, but I will summarize it here:
In the past, the NREMT and many States categorized EMTs as:
- EMT-FR (First Responder)
- EMT-B (Basic)
- EMT-I (Intermediate)
- EMT-P (Paramedic)
Not long ago, the NREMT and most States switched the names of these categories to:
- Emergency Medical Responder (EMR)– Formally EMT-FR
- Emergency Medical Technician (EMT)– Formally EMT-B
- Advanced Emergency Medical Technician (AEMT)- Formally EMT-I
- Paramedic– Formally EMT-P
Some people/websites still use the old terminology, and you will likely see or hear people still refer to EMTs as “EMT-Basics”, and sometimes will refer to Paramedics as an “EMT”, etc… Some States may still have some of these older categories of EMTs as well. Some states still recognized the EMT-Intermediate level, but do not issue new licenses at this level.
In general, If you want to work on an ambulance and be responsible for patient care, you will need to become licensed (at the minimum) as an EMT. Some states differ in what they will allow, so check your State’s EMS page below. Emergency Medical Technicians are the most common healthcare providers that work on ambulances. Once you have been licensed as an EMT, you can become either an Advanced EMT or Paramedic if you wish to do more training.
How Long Does it Take to Become an EMT
An EMT class takes approximately 1-4 months to complete, and some classes may only take 2-3 weeks to complete, depending on the program.
According to the National Emergency Medical Services Education Standards (that your State approved course will follow), the Course length is estimated to take approximately 48-60 hours for an Emergency Medical Responder (EMR), 150-190 hours for an Emergency Medical Technician (EMT), and 150-250 hours beyond the EMT requirements for an Advanced Emergency Medical Technician (AEMT). Paramedic Training will require approximately 1500-1800 hours of training.
Your EMT class will likely require that you do at least one ambulance ride along shift, and a clinical shift in an ER at a hospital. You may need a certain amount of patient contacts during these shifts.
There are some EMT/Paramedic courses that will let you take the classes online, and then take your practical exams in person. Make sure to check the links below for your State to help you find an EMT class in your area.
After you have passed your State approved EMT class (all the tests and practical exams), and become CPR certified (only takes a few hours), you will need to sign up to take the NREMT practical tests and exams.
EMT Practical Tests and Written Exams
In order to pass you State approved course and the NREMT exams, you will need to pass multiple written exams, and be able to pass a psychomotor exam.
EMT Cognitive Exam
The written exams for your State course and the NREMT (computer based test, or CBT) will have questions regarding:
- Scenario Based Questions
- Airway, Respiration and Ventilation
- EMS Operations
- Medical, Obstetrics, and Gynecology
- Trauma
When you take the Computer Based Test (CBT) for the NREMT, you will have 3 attempts to pass it (there is a fee to take it every time), and if you do not pass it after 3 attempts, you will need to take remedial training before you attempt it again.
EMT Psychomotor Exam
You will need to pass all of the psychomotor exam scenarios for your EMT class and the National Registry of Emergency Medical Technicians (NREMT allows 3 attempts before remedial training is required). You will be put in a scenario, and be responsible for patient care for:
- Trauma Assessment
- Medical Assessment
- Apneic Patient
- Oxygen Administration
- Spinal Immobilization
- Bleeding Control/Schock Management
- Cardiac Arrest/AED
- Joint Immobilization
- Long Bone Immobilization
You will not know what kind of Trauma or Medical patient you will get during your assessments. You will be required to assess the situation correctly, and intervene as necessary (example: assisting with an Albuterol inhaler during an allergic reaction with wheezing). You should be able to practice these scenarios multiple times during your State approved course.
How Long will My EMT Certification Last?
The National Registry of Emergency Medical Technicians (NREMT) certificate lasts for 2 years before you will need to recertify. For those of you that are certifying for the first time with the NREMT, you may not need to recertify for 3 years if you complete the certification process between July 1-December 31.
The first time I certified as an EMT with NREMT, I did it in late June; if I would have waited a few more days to finish the process, my certification would have lasted 1 more year before I needed to recertify.
Once you are certified with the NREMT as an EMT, you will be to apply for a State EMT license. You will need a State license to work as an EMT in that State (you cannot work with only a NREMT certificate).
For the record, I have been licensed as an EMT in 3 States, and as a Paramedic in 2. After I received my NREMT EMT certificate, getting my EMT license in different States was relatively easy (usually just an application, background check, and a fee), but each State is different with their requirements for receiving/maintaining an EMS license. Some States will also allow reciprocity without much difficulty, and others are a little more stringent.
Recertifying your EMT License
Some states do not require that you maintain your NREMT certificate in order to recertify your State license (others do), but I highly recommend that you maintain your NREMT anyway because you may decide to move to a different State in the future. EMT/Paramedic recertifications differ by State (usually every 2-4 years), and you will also need to recertify your NREMT certificate every 2 years, and your CPR license every 2 years. Recertification of your NREMT will require you to take a certain amount of Continuing Education, or you may recertify by taking an Exam.
